The short version.
We don't pay much more out of pocket but we make for that up in higher taxes and insurance premiums so that as a nation we pay a lot more than the rest of the world.
We don't really have better health outcomes than developed nations. Several others do healthcare better.
The rich are happier about healthcare than the poor.
Since we don't pay much out-of-pocket and we don't have good health care system to compare to, it is hard to convince people to change.
